Dental surgery

What is dental surgery?

The dental surgery is an area of dentistry that requires professional skills and knowledge. A dental surgeon performs the oral cavity area operations, but also supports other treatment procedures like endodontics, periodontics (periodontal treatment), implantology and dental prosthesis. The wide range of dental surgery procedures allows resolving the variety of problems. Every surgery is preceded by preparations necessary to avoid any complications. Types of surgeries:

  • Teeth extractions – the surgery that may incorrectly be considered as teeth being forcibly pulled out. In our office though we make every effort to perform the procedure smoothly using precise tools in order to dislocate and loosen the tooth’s root. The surgical forceps are being used only if the precise tools are not enough for the case. We stitch the post extraction wound, so it heals quickly and causes no trouble to the patient.
  • Surgical extraction of an impacted tooth – the extraction of third molars retained in the bone is much more difficult than regular extraction. It requires chiselling in order to cut and expose the gums in the tooth area. When the tooth is visible, the surgeon removes the bone plate using the drill and that way an access to the tooth is created. It’s nearly impossible to pull out the third molar in one piece, usually it has to be done partially. Thanks to local anaesthesia the procedure is painless, the patient may feel a little discomfort due to the pressure and expansion sensations. After the extraction the wound is being stitched and the patient is being instructed about after-treatment recommendations in order to maintain the proper healing process.
  • Hemisection of the tooth – the surgery is performed on multi-rooted teeth, damaged or broken molars, with periapical changes – when the endodontics is no longer possible. ·        As an inflammation usually is observed, it’s necessary to separate the tooth and remove its root along with the part of the crown – the healthy part of the tooth is being preserved as a base for prosthetic reconstruction. The rest of the roots remain untouched. The hemisection usually goes along with osteotomy – bone cutting.
  • The root apex resection – the indications for the procedure are: periapical changes of the teeth, teeth after incorrect root-canal treatment, unhealed bone inflammation after endodontic treatment or the inflammation of the root apex after the post and core and prosthetic crown filling. As a result of complications there may be cysts, caryopsis and periapical abscesses present. Those need to be removed immediately. The resection procedure is the root apex and inflamed tissue removal. This procedure allows saving the tooth from extraction. The treatment may be performed on permanent teeth when there are no contraindications (such as extensive inflammation, periodontitis, diabetes or kidney disease).
  • Sinus lift – lifting of the maxillary sinus bottom. The procedure increases the bone tissue amount allowing the implantation to the spots lacking bone tissue (most usually the back parts of the upper jaw). The bone substitute is being placed over the lacking bone tissue spot in order to provide bone growth conditions. This will enable the future proper implantation. The bone substitute is placed between the lower wall and the membrane of the maxillary sinus. Unfortunately it takes about 9 months before the implantation procedure is possible to be performed. It is worth waiting for, as thanks to the procedure, the implantation treatment is very efficient and makes no difference to the regular one.
  • PRF membranes – the platelet rich fibrin (autogenic membrane) received from fibrin clot centrifuged out of patient’s peripheral blood. It accelerates the healing process by stimulating the growth of new blood vessels. It also stimulates the missing bone tissue growth after the extraction, heals the pain, heals the alveolar bone surrounding the tooth socket when the socket is dry. It reduces the after-treatment swelling. What’s very important – the material is highly biocompatible and autogenic which makes the membrane very easy to adopt and lowers the risk of allergy.
  • Bone regeneration using bone transplants – the procedure of lacking bone tissue restoration. The missing bone precludes the implant from proper installation. Therefore the bone substitute is being applied and covered with a prohibitive membrane (in order to save the area from soft tissue growing in). It takes a few months before the applied material grows into the patient’s tissue creating the new bone base ready for implantation. The various materials used for this purpose:, bone autografts (the patient’s own bone material), homogeneous material (from the bone bank), xenograft – animal bone material or bone substitute. Each of those materials has some advantages and disadvantages. The most popular solution is Geistlich Bio-Oss – the beef bone tissue. Its structure and chemical composition is nearly identical to the human bone. Being applied to the missing bone area (as a granulate or block) it starts the bone tissue growth process. It takes 6 months to merge Bio-Oss with the patient’s bone structure and to create the new tissue. The foreign material slowly dissolves – it may take even 10 years and eventually the new bone tissue gains its stability.
  • Splitting – after the tooth extraction the alveolar bone atrophies. When it’s high enough but too tight for the implantation, it’s necessary to split it. The bone is being cut along the alveolar top in order to apply the implant into the slit. The rest of the gap in being filled with the bone substitute material and covered with a special membrane. This procedure enables even multiple implantations or creating a new alveolar bone shape.
